PCNL

PCNL (Percutaneous Nephrolithotomy) is a minimally invasive surgical procedure used to remove large or complex kidney stones directly from the kidney through a small incision in the back. It is considered the gold standard for treating stones that are too large (usually >2 cm), multiple, or not treatable by other methods like ESWL or URSL.

When is PCNL Recommended?

PCNL is recommended for patients with large, complex, or hard kidney stones that cannot be treated effectively with non-invasive procedures like ESWL or URSL. It is typically advised in cases of:

  • Kidney stones larger than 2 cm
  • Staghorn calculi (stones that occupy a large portion of the kidney)
  • Hard stones that do not break easily with shockwave treatment
  • Stones causing infection, obstruction, or kidney damage
  • Stones located in areas of the kidney that are difficult to reach with a ureteroscope

FAQ About PCNL Treatment

What is the recovery time after PCNL?

Recovery usually takes 7 to 10 days, during which patients are advised to avoid strenuous activities and drink plenty of fluids to support kidney function.

Will I need hospitalization after PCNL?

Yes, most patients stay in the hospital for 1 to 2 days post-surgery for observation and recovery.

Is PCNL a major surgery?

While it involves entering the kidney, PCNL is minimally invasive compared to open surgery. The incision is small (around 1 cm), and recovery is faster.

Are any tubes or stents placed after PCNL?

Often, a nephrostomy tube (to drain urine) or a DJ stent is temporarily placed to help the kidney heal and ensure proper urine flow. These are removed after a few days or weeks.

What are the risks or complications of PCNL?

Possible (but rare) risks include:

  • Bleeding

  • Infection

  • Injury to surrounding structures

  • Residual stones requiring a second procedure

Will the stones come back after PCNL?

PCNL removes the current stones, but it doesn’t prevent future stone formation. Preventive care, including dietary changes, increased water intake, and regular follow-ups, is important to avoid recurrence.
